Home & Community Care (HACC)

MMIGP has an Aboriginal Liaison Officer who works closely with Local Government HACC Service Providers to provide a culturally appropriate and easily accessible HACC service to eligible clients in the EMR.

Who is eligible for MMIGP Home and Community Care (HACC) Program

  • Elderly with a disability and their carer 
  • A Young person with a disability and their carer

HACC can support you to remain independent to live in your community by;

  • Respite Care
  • Home Care 
  • Property Maintenance
  • Arrange Community Transport
  • Food Services
  • Personal Care
  • Home from Hospital
  • Home & Hospital Visits

MMIGP HACC Aboriginal Liaison Officer provides;

  • Assistance for HACC clients to link into other services
  • Advocates for HACC clients
  • Support while receiving medical treatment
  • Family support linking  into services
  • Personal Introduction to  HACC services

MMIGP Elders Activity Group Provides

  • Social Support
  • MMIGP Elders PAG –Community & group activities ,short trips, cultural activities 
  • MMIGP Transport – Helps Elders to get out to Elders Activities and community events
