The Aboriginal Health Promotion and Chronic Care Program (AHPACC) is designed to support both The Aboriginal Community Controlled Health Organisations and the Community Health Services (CHS) to work in partnership to increase access to primary health care and services by Aboriginal people. The goal of this program is to improve health outcomes for Aboriginal Victorians including those with chronic disease or at risk of developing chronic disease, through health promotion, prevention and early intervention and ongoing care.

Talk to our Health Promotion Officer or the Facilitator (Nurse) on how to access health services in your area including:

  • Doctors
  • Dieticians
  • Dentists
  • Physiotherapist
  • Podiatrists and Foot care
  • Social Workers
  • Health Equipment
  • Diabetes Educators
  • Councillors
  • Speech Therapists.
